Covid-19 Health Declaration

It is important to us that we keep you, our staff and our community safe. Please fill out the following form
in order to proceed with your booking with Beauty by Sophie

Do you have any new or worsening respiratory symptoms including one of the following: new or worsening cough, sneezing, and runny nose, sore throat, fever, temporary loss of smell or altered sense of taste, shortness of breath?
Do you have any of the less common symptoms such as: diarrhea, headache, muscle aches, nausea, vomiting, malaise, chest pain, abdominal pain, joint pain, confusion or irritability – where there is no other likely cause or diagnosis?
Are you currently required to self isolate and/or waiting a Covid-19 test result?
Do you have a confirmed diagnosis or probable case of Covid-19
Artists are unable to follow social distancing when working on a client. Do you consent to your artist working close to your face to carry out your service? Our staff will be wearing masks.

Due to government mandates we are only allowed to provide our services to fully vaccinated clients at this time. You will be required to show your vaccine pass at your appointment and have it scanned with the NZ Verifier App.
